Can I fill out my information online before my appointment?

At Friendly Smiles Dental Care, we want to ensure patient privacy. It is with that in mind, that we offer our Secure Patient Portal to fill out all your forms before your appointment.

What information do you need at my first appointment?

When you come, be sure to think about any previous dental health history. Information or records containing details on medical conditions and medications are very helpful. Also, don’t forget your dental insurance information needs to be obtained before the initial visit, so that we can ensure eligibility and coverage. We also recommend you bring your Driver’s License and/or SS card if possible.

Can I schedule multiple family members at once?

We ask that you only schedule up to 2 people at one time. We do this to ensure you get the best available care possible and that every visit includes a comprehensive exam and time with a member of our team.

What’s your policy for rescheduling appointments?

We require 48 hours’ notice to reschedule appointments in order to avoid a fee. If an emergency should arise we ask for verification/documentation. This helps us ensure all of our patients have availability and accessibility to our team and doctors. Thank you for your help in keeping your appointments!
